Hope House
Hope House is a 12-bed men’s board and care home for clients who are able to step down to a lower level of care but who still benefit from additional support. Many residents are still conserved, and some have been able to get off of conservatorship. Hope House’s dedicated staff are on site 24 hours a day, 7 days a week, to proactively educate residents on life skills as well as assist with needs as they arise, be it medication management and personal care, or conflict resolution and crisis intervention.
At Hope House, residents learn to care for themselves and their living environment as they work communally to maintain the common areas of the house. Meals are prepared by staff and provide residents with opportunities for nutrition education and training for safely working in the kitchen. Staff provide interactive learning groups where residents gain skills in money management, goal setting, safe coping, and other life skills that build both confidence and independence.
All residents are case managed through Telecare Sonoma ACT and provided by an individualized treatment team with further services in that program.

Potential residents must be:
- Male
- 18 years and older
- Clients with Sonoma County Behavioral Health/Telecare Sonoma ACT
- Referrals are made by the Telecare or the SCBH case manager to the Linkage meeting
- Ambulatory, we are not able to take clients with oxygen tanks or who require 24 nursing support.
