Case Managed, Transitional Housing
The Bridges program is a 10-bed transitional home in Santa Rosa. This goal of this program, in partnership with Sonoma County Behavioral Health, is to support residents as they plan for and make decisions about what they want for their future. Residents meet with on-site staff to review daily plans, set goals, and participate in case management and support groups. They learn about money management, housing opportunities, and accessing community resources such as local food banks and public transportation.
Residents begin with building relationships within the Bridges community, while compassionate staff support them in developing important community connections as they begin volunteering, working, or going back to school. Staff also actively support and encourage involvement in AA, 12 Step groups, Seeking Safety, NA, and/or dual recovery support groups.
Potential residents must be:
- 18-59 years old
- Clients with Sonoma County Behavioral Health
- Referred to CSN through the SCBH Linkage committee
- Funded
