Hope House

Hope House is a 12-bed men’s board and care home for conserved clients who are able to step down to a lower level of care but who still benefit from additional support. Hope House’s dedicated staff are on site 24 hours a day, 7 days a week, to proactively educate residents on life skills as well as assist with needs as they arise, be it medication management and personal care, or conflict resolution and crisis intervention. This is in addition to care provided by an individualized treatment team at Telecare Sonoma ACT.

At Hope House, residents learn to care for themselves and their living environment as they work communally to maintain the common areas of the house. Meals are prepared by staff and provide residents with opportunities for nutrition education and training for safely working in the kitchen. Staff provide interactive learning groups where residents gain skills in money management, goal setting, safe coping, and other life skills that build both confidence and independence.

Eligibility

Potential residents must be:

  • Male
  • 18-59
  • Clients with Sonoma County Behavioral Health
  • Referred to CSN through the SCBH Linkage committee
  • Funded
